Transitions Centers Inc. gives people the ability to increase their confidence and competence in community and life pursuits. This is accomplished through the variety of services that we provide our Members. The services that we currently provide is Day Habilitation, Supported Community Employment, Individualized Supports for non-residential members and Transportation Services for our Day Habilitation Members.

Transitions Centers provides services to the neuro-diverse population. This includes, but is not limited to people diagnosed with:

· Developmental Disabilities

· Intellectual Disabilities

· Autism Spectrum Disorder

· Generalized Anxiety Disorder

· Psychiatric Disabilities

· Prader Willi Syndrome

· Deafness

· Vision Impairment/Blindness

We want to help people to further their independence. Independence can mean different things to different individuals. Examples could include the range from advocating for one’s self to getting a dream job to living independently.

  • The Transitions ASL Developmental specialist assumes primary responsibility for each member’s goals and acts as an integral part of the member’s team to ensure that all needs and goals identified are addressed and implemented.
  • The ASL Developmental Specialist assumes responsibility for each member’s goals while communicating primarily through sign language.
  • Provides direct support of members in a wide range of community activities.
  • Ensure safety of member on community outings
  • Drive selected group members to community activities
  • Ability to participate in all activities with members
  • Ensure that the member’s Service Plans are implemented on a consistent basis with attention to quality and member satisfaction.
  • The ASL Developmental Specialist is responsible to ensure the member as identified in the Plan makes effective progress/growth and that any obstacles that may arise are addressed.

Job Type: Full-time, 30 hours per week

Hours: 8:30am-2:30pm Monday-Friday-no weekends

COVID-19 considerations:
All staff and Members are required to wear a mask. Daily screening of all Members and staff. Social distancing is encouraged.
