Case Manager

Job Description
Full-time 40 hour position responsible for the coordination of all services by consumers on assigned caseload. The Case Manager works closely with the Manager, clinical staff, and Developmental Specialists and Aides in the development and implementation of Day Habilitation Service Plans and Individual Service Plans (as applicable).
 
QUALIFICATIONS: At least three years' experience and/or training in working with individuals with developmental disabilities and demonstrated working knowledge of principles of normalization. Independent means of transportation, a valid driver's license and a satisfactory driving record required. Acceptable criminal background check required. Experience as a Case Manager, Personal Care Attendant or related human services position a plus. Initiative, flexibility, ability to work independently, good time management skills, and/or Bachelor's degree in relevant human service field preferred.
